New Patient Registration


If you live within our practice area  (click here to see map)  you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof of your address, e.g. recent utility bill.  You will also be required to provide some photographic ID

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. If you wish, you can print off the forms and fill them out and bring them on your first visit to the practice.

All new patients over the age of 5 will be offered a health check with the Practice Nurse.  

Medical treatment is available from the date of registration.

Please contact reception for further information. 

Please note that you will be registered with the practice and not an individual GP. For administrative purposes you will be allocated to a named doctor; however you can at any time express a preference for a particular doctor for either all of your medical needs or on a case-by-case basis.

Changes to Personal Details

If you change your name, address or contact details please inform the practice as soon as possible. You can complete the online form here
